Agreement and Certification Form for the Rhode Island Business Plan Competition

The Agreement and Certification Form  must be completed for applications to be considered complete. This form states that the plan being submitted to the Rhode Island Business Plan Competition is the original work of the applicant(s), that the applicant(s) will abide by the rules of the competition, and that the applicant(s) release the co-organizers of the competition and their representatives from any liability.

Please do the following:

2. Complete this form.
3. Have all applicants sign it.
4. Mail it by April 1, 2013, or hand deliver, to:
